Coralville Employee UPASS Replacement Form

A sequence of Coralville (CV) UPasses will expire at the end of the day on Dec. 31, 2023. If your Employee CV UPass number is between CV00500-CV02100 it will need to be replaced.

US Mail option:

Please complete the form below by Friday, Dec. 1 if you would prefer to have the UPass mailed to you. Once you receive your new UPass the old one can be discarded.

Pick-up at WCTC option:

Please stop into the West Campus Transportation Center to swap your current UPass for a new one between Monday, Oct. 16 and Friday, Dec. 29. Our office hours are Monday-Friday 7:30 a.m. - 5 p.m.

Note: If no action is taken by the UPass member then your UPass will automatically expire at the end of the day on Dec. 31 and your last deduction will come out of the Jan. 1 paycheck.
