Van Pool Payroll Deduction Authorization

If you elect to participate in the Van Pool Program, you agree to:

Grant permission to deduct van pool fees from your University payroll or pay by other means if deduction is not possible.
Payroll deductions are one month after fees are charged (for example, September 1 payroll deduction is for August van pool fees).
Any Van Pool related fees and sanctions may be payroll deducted if they are not paid after the initial billing.
If you discontinue participation, this authorization for van pool fees payroll deduction will terminate when all due van pool charges have been deducted.
